Nightmare Reaper, where you play as a girl put into an insane asylum to try and fix her mind, The girl is normal and likely okay when awake. but in the world of her dreams she is running around as a game character causing carnage and destruction.

You can get weapons in the game, and keep one weapon at a time between levels. All the rest are sold off. Then to power up you play her hand held wither in her dreams or in the real world. And in the Real there is a puzzle to be solved.
