Doom 2: The Master Levels - Map 20: The Express Elevator To Hell (teeth.wad)

Welcome to the Express Elevator to Hell (abbreviated T.E.E.T.H. for short), the most cramped map in the Master Levels, with two major trouble spots. First, the intro where you're surrounded by Chaingunners and Hell Knights. Once you're able to get your foot in the door, the rest of the map isn't overly terrible...until the exit room. There is a secret exit in this level, and the only way to get to it is through a technique called the Arch-Vile Jump, where you use the vertical launch of an Arch-Vile's attack to reach an otherwise unreachable ledge. There's an invulnerability sphere in the room to make this less painful, but the problem is you have to get past one of the Arch-Viles to reach it and you might get trapped by the other Arch-Vile on your way back out, and if you kill both of them, that's it. No secret exit for you!

I've found the easiest way to do this is kill the Arch-Vile on the left first. Then lure the other Arch-Vile around to your side. Hit the switch, lower the elevator, kill the Baron of Hell, open the regular exit, lower the elevator, walk off to the other side, grab the Invulnerability, then take the Arch-Vile Jump, kill the stragglers from above, then open the key doors and get out. Speaking of keys, it's interesting to note that you don't need any of the keys to get to the regular exit, only for the secret exit.
