Milk thistle is a member of Carduus marianum family. It has a long history as a medicinal plant, it has been used for the treatment of different diseases such as menstrual disorders, digestive problems, liver and gallbladder disorders. European physician Nicholas Culpepper was the first person who first noted that milk thistle was excellent in opening the obstructions of the liver. The active compound of this plant is silymarin, which is known for its hepatoprotectant and antioxidant activities. For many years, it has been used as a hepatoprotectant but the mechanism of action is not completely known. This could be attributed to its antioxidant, immunomodulatory, antifibrotic, antiproliferative, and antiviral properties. Silymarin is also a lipid-lowering agent. Silymarin inhibits the development of diet-induced hypercholesterolemia and at the same time causes in increase in high density lipoprotein-cholesterol, which can be considered to be of benefit. Currently, reports suggest that milk thistle can exert beneficial effects in patients with non-alcoholic fatty liver disease. In addition, milk thistle also protects cell membranes from free radical-induced damage and directly inhibits radical formation.
