Is Polio back? Why is this crippling disease in news?

Polio is considered eradicated from USA since 1979 and from India since 2014. This year some cases of Polio have been found in New York state, London and Jerusalem.
Is Polio back? Why is this crippling disease in news and what is its history?

Many, if not all, of these new polio cases can de traced trace directly back to vaccines that can, and often do, cause polio.
