He was an adopted son of Cao Cao. There are two accounts of Cao Zhen's origins. The first, from the Records of the Three Kingdoms (Sanguozhi) and Wei Shu (魏書; by Wang Chen), claimed that Cao Zhen was a distant younger relative of Cao Cao.

The second account, from the Weilue, mentioned that Cao Zhen's original family name was Qin (秦), and that he was adopted into the Cao family. In this account, Cao Zhen's father was one Qin Bonan (秦伯南), who was a close friend of Cao Cao.
