If you are coming from an EU country you don't need any health forms or covid documentation. If you are traveling from outside of the EU and entering Spain you need the Spth form and QR code which you can find in the link below. As always, be sure to do plenty of research (I'm not perfect so the information here is really what I have found through research, so make sure to double check, because I cannot guarantee the accuracy of the information.

There are no restrictions between Spain and Gibraltar related to Covid of Covid testing to enter Spain or enter Gibraltar via the land border.
