Os jogadores controlam Marco e Tarma, ambos membros da força de operações especiais Team Peregrine Falcon (comumente conhecido como Team PF), e lutam por fases após fases de ação intensa. O objetivo é tentar derrubar o General Morden e reconquistar a arma roubada, o Metal Slug. Os jogadores devem passar por ondas de inimigos e máquinas (ao mesmo tempo em que saltam sobre quaisquer obstáculos no caminho) para avançar pelas fases. Não é tão desesperador quanto pode parecer - armas como metralhadoras pesadas, espingardas, lançadores de foguetes e lança-chamas, bem como o próprio Metal Slug (que aparece com frequência no jogo), tornarão a batalha mais fácil.

Players control Marco and Tarma, both members of the special-ops force Team Peregrine Falcon (commonly known as Team PF), and battle their way through stage after stage of intense action. The goal is to try to overthrow General Morden and win back the stolen weapon, the Metal Slug. Players must blast through waves of enemies and machines (while also jumping over any obstacles in the way) to advance through the stages. It's not as hopeless as it might sound, though - weapons such as heavy machine guns, shotguns, rocket launchers and flamethrowers, as well as the Metal Slug itself (which appears frequently in the game), will make the battle easier.

プレイヤーは特殊部隊チーム・ペレグリン・ファルコン（通称チームPF）のメンバーであるマルコとターマを操作し、激しいアクションのステージを次々と進んでいきます。目標は、モーデン将軍を打倒し、盗まれた武器であるメタルスラッグを取り戻すことです。プレイヤーは、ステージを進むために、敵やマシンの波を爆破する必要があります (途中の障害物も飛び越えます)。絶望的ではありませんが、ヘビー マシンガン、ショットガン、ロケット ランチャー、火炎放射器などの武器やメタル スラッグ自体 (ゲームに頻繁に登場します) が戦闘を容易にします。
