Queen's Gambit | How to Throw a Completely Winning Endgame

It all started with the opening. I guess I wasn't fully warmed up from my blitz games but I played an odd first move and went into the Queen's Gambit Declined on accident. That being said, the game went very well after that all the way until the end. I was completely winning, but I had a bishop and knight for a rook. You have to coordinate those pieces well to materialize your advantage... and well... I didn't haha.

I was completely winning until the last 2 moves, so put on your calculation hats and see if you can do a better job than me.
