How do you help an addicted loved one when another family member keeps enabling?

Dear Amber...What do I do if my addicted loved one... Has severe medical problems but keeps using? Becomes abusive when drinking? Has another family member that keeps enabling? Has bad self-esteem and won't talk to a counselor? Has severe depression and anxiety? Wants to go to a methadone clinic? No one else know about the problem? Has caused trauma to our children? Tries to emotionally blackmail me? Isn't doing what they said they would?
