➡️ What is Capillary Blood Gas Sampling?
A Capillary Blood Gas (CBG) is the process of puncturing and collecting a blood sample in infants from the capillary bed. This allows the practitioner to analyze the neonate’s ventilatory status by observing the pH and PaCO2 values. The lateral area of the heel is the preferred puncture site when collecting a capillary blood gas sample. You could potentially use the infant’s fingertip, toe, or earlobe if a heel puncture attempt is unsuccessful. With that said, these are not the preferred sites. The heel is the preferred and typically most effective site to stick in infants because it is highly vascularised.

➡️ Capillary Blood Gas Procedure
Before puncturing the infant, you first need to warm the heel to 42°C for 10 minutes. Clean the site with an antiseptic solution and then puncture the skin less than 2.5 mm using the lancet. After puncturing the skin, wipe away the first drop of blood and observe for free flow. You do not need to squeeze or milk the puncture site. Then, you can fill the sample tube with 75–100 microliters of capillary blood. Seal the sample and mix well, and be sure to place a sterile bandage over the puncture site. The sample should be analyzed within 10–15 minutes if left at room temperature.
