Dear God,
I seek your infinite knowledge
and your infinite ability
I seek your great favors on us
You are able while we are not
You are All-Knowing while we are not
And you have knowledge of the unseen

Dear God,
I will make this decision
if You know that this decision
is good for me in Dunia and Hereafter,
then decree it for me
and facilitate it for me with ease
then bless its results

But if you know that this decision
is bad for me in Dunia and Hereafter,
then get it away from me
and get me away from it
and decree for me
what is best for me
then make me satisfied with it

Istikhara in Arabic:

اللهُمَّ إنِّي أسْتَخيرُكَ بعِلْمِكَ، وأسْتَقْدِرُكَ بقُدْرَتِكَ، وأسْألُكَ مِنْ فضلِكَ العَظِيم، فإنَّكَ تَقْدِرُ ولا أقْدِرُ، وتَعْلَمُ ولا أعْلَمُ، وأنْتَ عَلاَّمُ الغُيوبِ، اللهُمَّ إنْ كُنْتَ تَعْلَمُ أن هذَا الأمرَ -ويسمي الشيء الذي يريده- خَيرٌ لي في دِيني ومَعَاشي وعَاقِبَةِ أمْري عَاجِلهِ وآجِلِهِ فاقْدُرْهُ لي ويَسِّرْهُ لي، ثمَّ بَارِكْ لي فيهِ، وإنْ كُنْتَ تَعْلَمُ أنَّ هذا الأمْرَ شَرٌّ لي في دِينِي ومَعَاشي وعَاقِبَةِ أمري عَاجِلِهِ وآجِلِهِ فَاصْرِفْهُ عَنِّي، وَاصْرِفْنِي عَنْهُ، واقْدُرْ لِيَ الخَيْرَ حَيْثُ كَانَ، ثُمَّ أرْضِنِي بِهِ
