Doom Zero (Ultra-Violence Plus 100%) - Map 23: Atmos Caves

Map 23 of Doom Zero is a uniquely designed map, where the exit is right where you start, but you have to traverse three separate sections with each section ending with a switch that raises one stair toward the exit. Each section isn't overly difficult, though the box maze in one section with all the Specters on speed can trip you up, as well as the horde of Cacodemons and Pain Elementals guarding the switch at the end of said section.
