A new report from the Justice Department’s Office of the Inspector General detailed systemic failures that lead to the 2019 death of Jeffrey Epstein, including how the disgraced financier was able to hoard prison linens and other supplies before he was found hanging in his jail cell. The 128-page report concludes that mismanagement and other failures by the Bureau of Prisons and its since-shuttered Metropolitan Correctional Center gave Epstein the means and opportunity to die by suicide.
