Doom: Revolution! (Unity Add-On) - Map 21: Hausterium (UV-Max)

Map 21 of Revolution is a very short map, though it can be tricky if you're not careful with the crushers in the narrow corridor with the Cacodemons and Revenants. Most of this map kind of looks like a monster's insides, which gives off a real creepy feel.
