Short call ITM, how to take profits: poor man's covered call

For a poor man's covered call you buy a call deep in the money and month into the future. Then you sell out of the money calls against it and pocket the premium. This only works if the short calls continue to expire worthless. But what do you do if you know it won't expire worthless?
