If we really pay attention to people we are meeting or who we just haven't felt right around and known from a distance for even years there is really a number 1 tell if someone is a Cluster B Personality Disordered person or not.

How can we know on a first date? How can you know when you first meet someone? What is up with that person at work, that neighbour? What is up with that acquaintance? What is up with that relative or friend? There is one major sign that can give them away. Perhaps not immediately if they are good actors, but soon after.
