In the 2000's Toyota had to recall over 9 Million cars.

Due to, floormats?

In this time period, Toyota fitted all of thier cars with removable floormats.

To make them easy to take out and clean.

But they didnt expect the floormats would move around on the floor as you step
on them eventually sliding over the accelerator resulting in the car rapidly
accelerating

Meaning the car would start driving as fast as possible leaving many people
unable to stop.

Resulting in thousands of crashs and around 89 deaths.
