Railroad employee failed to properly secure the handbrakes on train cars prior to a crash in downtown Indianapolis, according to new report from Federal Railroad Administration

The train crash last August sent chemical spilling out onto the tracks. The rail cars actually rolled away from CSX's State Street rail yard and slammed head-on into a locomotive heading through downtown.
