Max Payne 3 continúa la historia del ex detective de Nueva York, Max Payne. Aún atormentado por los recuerdos de su traumático pasado, Max inicia una nueva vida trabajando en seguridad privada junto con su amigo Raúl Passos, protegiendo a la familia Branco en São Paulo, Brasil. Cuando las bandas fijan su objetivo en la familia bajo su protección, Max se ve obligado a luchar para salvar a sus clientes y limpiar su nombre, en un intento de liberarse, de una vez por todas, de los demonios que le han torturado durante demasiado tiempo. Al principio del juego, el Comando Sombra secuestra a Fabiana, la esposa de Rodrigo Branco de una fiesta privada, reclamando un rescate de tres millones de dólares. Max tendrá que arreglárselas para conseguir salvar a la esposa de su jefe. Max Payne 3 continues the story of former New York detective Max Payne. Still haunted by memories of his traumatic past, Max begins a new life working in private security alongside his friend Raul Passos, protecting the Branco family in São Paulo, Brazil. When gangs set their sights on the family under his protection, Max is forced to fight to save his clients and clear his name, in an attempt to free himself, once and for all, from the demons that have tortured him for too long. Early in the game, the Sombra Command kidnaps Fabiana, Rodrigo Branco's wife, from a private party, demanding a ransom of three million dollars. Max will have to manage to save his boss's wife. Max is forced to make a deal with the kidnappers, and after it goes wrong, and they lose the money, he goes in search of Fabiana, to the favelas of Nova Esperanza, not knowing what awaits him there.
