What is the Separation of Church and State?

Every once in a while, we hear the phrase "separation of church and state" get tossed into the political conversation. It seems to be universally accepted that this idea is part of American history, but few realize where this phrase came from. Is this phrase even in any of our founding documents? And if it isn't, does this idea still have any significance to us today?
