Prostate cancer can be heterogeneous, and not all subtypes produce prostate-specific antigen (PSA). The typical PSA-producing cells are usually adenocarcinoma cells, which make up the majority of prostate cancers. However, there are non-PSA-producing variants, such as:

Neuroendocrine prostate cancer (NEPC)
Small cell carcinoma

This is something I have to look further into.
