On July 21, 2025, the U.S. Department of Health and Human Services (HHS), led by Secretary Robert F. Kennedy Jr., announced sweeping reforms to the nation’s organ transplant system after a federal investigation uncovered alarming “signs of life” in patients during organ procurement.

The Health Resources and Services Administration (HRSA) reviewed 351 cases of authorized but incomplete donations, finding:
- 103 troubling cases, including 73 patients with neurological signs incompatible with donation
- 28 patients potentially not legally deceased at the start of organ procurement

These findings point to major failures in neurological assessment, cause-of-death classification, consent processes, and team coordination.
