"Lies": This is supported by the investigation's findings that OceanGate's CEO misrepresented the Titan's safety and that the company was found to have downplayed, ignored, and even falsified key safety information to avoid scrutiny.

"Errors": The report details numerous critical failures. The Coast Guard found that the disaster was "preventable" and pointed to a "critically flawed" safety culture, inadequate design and testing processes, and a failure to follow established engineering protocols for safety and maintenance.

"Laws Broken": The report highlights how OceanGate actively evaded regulations. It states the company "strategically created and exploited regulatory confusion and oversight challenges" to operate outside of established safety protocols.
