Completed Ending 1, 2, 3, & 5. On path for Ending 4.
I started my 4th Playthrough during the last episode after finishing Ending 3.

I was not able to pick up Sakuko's Diary #7 entry in that episode. I tried doing some searches. A few sources mention how the game checks your documents and decides which to spawn and that NG++ may reset the glitch. Three documents spawn in the same spot. The game performs a check based on which ones you've already collected for that spot. But it can fail, not spawning the document you need because it checked for a file you already have. This spot checks for document 3, 5, 7. Some others have mentioned they've had the same glitch with #5. So the document check for this spot is bugged.
Allegedly, when you enter NG++ mode it resets the check, so it should spawn the next document you're missing from those three.
If anyone has encountered this fix or another fix, please leave a comment.
