For actual isolation of your rear deltoid, pronate your grip. The palms down version allows the line of pull against gravity to shift towards the back of your shoulder. A neutral grip, when lying on an incline bench, involves much more lateral deltoid. Pronated grip puts the rear delt at a much higher mechanical advantage to move the weight in your hand behind you in horizontal abduction. This concept apllys to machines and cable machines as well.
