The Appendix.

The appendix is a narrow, vermiform diverticulum arising from the posteromedial aspect of the caecum, typically located at the confluence of the taeniae coli.

Histologically, it contains prominent gut-associated lymphoid tissue (GALT), particularly in childhood and adolescence, suggesting a role in mucosal immunity.

Increasing evidence indicates that the appendix may function as a reservoir for commensal microbiota, facilitating microbial reconstitution of the colon following episodes of enteric infection.

Although anatomically small and often regarded as vestigial, the appendix represents an immunologically active niche within the gastrointestinal tract.

Pathology of the appendix most commonly reflects luminal obstruction due to lymphoid hyperplasia, faecoliths, parasites, stricturing, or neoplastic processes.

Obstruction results in intraluminal pressure increase, venous congestion, mucosal ischaemia, and subsequent bacterial overgrowth, culminating in acute appendicitis. Progression may include transmural necrosis, gangrene, and eventual perforation, with spillage of purulent or faeculent material into the peritoneal cavity.

Complications—ranging from localised peri-appendiceal abscess formation to diffuse suppurative peritonitis and sepsis—underscore the clinical significance of appendiceal disease. Thus, the appendix, despite its diminutive form, occupies a disproportionately important place in abdominal pathology.

The world record for the longest appendix removed is 26 cm, which was removed from Safranco August of Croatia in 2006 during an autopsy.
