【74】感染症検査法のゴールデンスタンダードを考える - 大橋眞

検査法のゴールデンスタンダードは、病気の指標として、最も信頼が出来るものであり、感染症においては、病原体の数を直接的に表しうるものになります。これは、病原体が増えることが病状につながることから、出てくる考え方です。利便性などの理由により新たに検査法を確立する場合には、ゴールデンスタンダードとなる検査法と比較して、相関が良ければ、この検査法は有望なものになり得るわけです。現在抗体検査や抗原検査を確立するにあたり、PCR法がゴールデンスタンダードとして用いられています。果たしてこれは、適正な方法でしょうか。
　実は、現行のPCR法は、症状と関連付けて確立されたものではありません。症状を出すレベルのウイルス数と比較して、非常に少ないレベルのウイルスを検出していることがほとんどというのが実態です。症状を出すレベルの1０万分の１程度のウイルス数を問題としているからです。すなわち感度を最大限にして、症状とは無関係な検査になっているためです。これでは、検査法として意味をなさないわけです。すなわちゴールデンスタンダードになり得ないのです。症状と関連付けが出来る適正なレベルでPCRを実施しないと無意味な検査になるわけですが、症状と関連付けて考えられるウイルス量が検出される例は、極めて少ないために、これを病原体と呼べるようなものではないということです。このような現行のPCR検査の問題点に気づく必要があります。

Actually, the current PCR method has not been established in relation to the symptoms. The reality is that most viruses are detected at a very low level compared to the number of viruses that cause symptoms. This is because the problem is the number of viruses, which is about 1 / 100,000 of the level that causes symptoms. In other words, the sensitivity is maximized and the test is independent of symptoms. This doesn't make sense as an inspection method. In other words, it cannot be the golden standard. If PCR is not performed at an appropriate level that can be associated with symptoms, it will be a meaningless test, but since there are very few cases where the amount of virus that can be associated with symptoms is detected, this can be called a pathogen. It's not a symptom. You need to be aware of these current PCR testing issues.
