My Brother is Asexual But He Got Married Anyway

Question: My brother is asexual but he got married 15 years ago. After my sister-in-law got back from their honeymoon she told my family, “My husband doesn’t desire me.” They ended up having 3 kids so at some point they had sex at least 3 times. Fast forward to today: she just got back from a 60 day visit to a facility in Arizona for self harm issues. The place called my brother and recommended she be committed, but he refused to even consider that. He told them, “You’re supposed to fix her.” So my question is, does she really need to be committed like that place said and also, could she and my brother be mixed up in some kind of dysfunctional dynamics?
