Press the Option button
Move over to the pouch
Select which slot to use
Press Square to remove the current item
Press Triangle to select the new item
Press X to confirm the new item
Press Circle to confirm and move back to the option screen
Press Circle again
Hold Triangle to bring up the short-cut menu and hit the direct key for the needed item, in this case, up.
Or press Triangle and the needed direction at the same time.
